How do you stop condensation forming?

We have double glazed windows which are also fitted with secondary glazing. we have a very large patch of black mould on our living room wall. We have checked the outside of the house for anything that could be causing a problem but the outside seems fine. our neighbour suggested it's probably condensation but didn't know how to cure it, do you?
